The “Practice Leaders in Medical Homes” online course will be free to practice leaders seeking qualification to participate in the Medicare Medical Home Demonstration. What does this mean?
Shortly after the Centers for Medicare & Medicaid Services (CMS) announce the sites for the Medicare Medical Home Demonstration, eligible practices can apply to participate in the demonstration by completing the online application form available at http://cmsmedicalhome.com. CMS will select a mix of small and large, urban and rural practices to begin the demonstration's qualification phase, during which practices complete the PPC®-PCMH™-CMS survey tool and document their Medical Home capabilities.
When a practice receives notification from CMS that it has been selected and may begin the demonstration's qualification phase, the “Practice Leaders in Medical Homes” online course will be available for free to the practice leaders of those selected practices. |
