This 6-week, 40 hour web-based Guided Care Nursing course and examination lead to the American Nurses Credentialing Center’s new Certificate in Guided Care Nursing. The course features core and supplemental modules, live webinars and simulations, and support from expert faculty from the Institute for Johns Hopkins Nursing.
A nurse who successfully completes this course will be well positioned to work in many "medical homes" and other organizations that provide comprehensive, coordinated, continuous care to their patients, including those with chronic conditions who require complex services. Nurses who want to learn new skills and be more effective in caring for older adults with multiple chronic conditions may also be interested in this course.

Guided Care provides many aspects of a patient-centered medical home for the growing number of older adults with complex health needs. In Guided Care, a registered nurse based in a primary care office works closely with three to four physicians and uses health information technology to provide comprehensive care for up to 60 chronically ill older adults.
