The web-based Guided Care Nursing course and examination lead to the American Nurses Credentialing Center’s new Certificate in Guided Care Nursing. The course features core and supplemental modules, live webinars and simulations, and support from expert faculty from the Institute for Johns Hopkins Nursing.

Guided Care provides many aspects of a patient-centered medical home for the growing number of older adults with complex health needs. In Guided Care, a registered nurse based in a primary care office works closely with three to four physicians and uses health information technology to provide comprehensive care for up to 60 chronically ill older adults.
