What the Experts are Saying…
“This book is an eminently practical and readable manual to help health care organizations fully consider Guided Care, and, if interested, implement it. It covers every aspect of the intervention from hiring nurses to assuring financial viability."

Ed Wagner, MD, MPH
Director of the MacColl Institute for Healthcare Innovation
Group Health Cooperative of Puget Sound

"Boult and his colleagues have created a model to improve the care of frail older people that has taken the best of many approaches and weaved them into one seamless package. Guided Care fills in the gaps in care coordination that the primary care physician does not have the time to provide."

David Reuben, MD
Director, Multicampus Program in Geriatric Medicine and Gerontology
Chief, Division of Geriatrics; Archstone Professor of Medicine
David Geffen School of Medicine at the University of California, Los Angeles

"Physicians and nurses caring for complicated elderly patients need to read Guided Care: A New Nurse-Physician Partnership in Chronic Care. This book provides a detailed program for improving the care of these challenging patients. Finally we have a primary care model - Guided Care - that integrates care and reduces costs for patients with complex health needs, the ten percent of people responsible for seventy percent of health care costs. If Medicare invests in primary care practices to pay for the additional expenses of Guided Care, Medicare will save billions of dollars from fewer hospital admissions."

Thomas Bodenheimer, MD, MPH
Adjunct Professor
Department of Family and Community Medicine
University of California; San Francisco General Hospital

Guided Care provides many aspects of a patient-centered medical home for the growing number of older adults with complex health needs. In Guided Care, a registered nurse based in a primary care office works closely with three-four physicians and used health information technology to provide comprehensive care for up to 60 chronically ill older adults. Click here for more information about Guided Care.
