Certificate Requirements

The target audience for this course is physicians, practice administrators, and other practice leaders who wish to transform their practices into medical homes. The course provides practice leaders with an awareness of the competencies needed to facilitate a medical home.  

It is an asynchronous course that begins with a short introduction followed by nine one-hour modules. Each module (except for the introductory module) is comprised of six parts:  

  1. Self-assessment
  2. Short reading
  3. Narrated presentation
  4. Case study/video vignette
  5. Final assessment (passing score of 70% or higher required)
  6. List of additional resources

In order to print out the certificate at the end of each module, you must complete parts 1-5 and then complete the evaluation survey at the end. If the system does not register completion of all items, the certificate will not print out.  

Remember, you are responsible for the following: 

  • Reviewing all of the information in each module,
  • Completing each assessment and survey, and
  • Tracking your progress (you may use the progress-tracking page for this purpose, located under the My Trainings tab).
