Module 6: Supporting Patient Self-Management within Medical Homes

Upon completing this course, you will be able to:

  • Define patient self-management and distinguish it from patient education.
  • Describe a five-point strategy to help patients gain self-efficacy and change their lifestyle behaviors.
  • Discuss how self-management support can be provided by a medical home.
  • Identify three resources to assist patients with self-management.
  • Identify three resources to assist health professionals to support patients’ self-management.

The cost of this module is $15 per person. The module will take up to 60 minutes to complete and includes:

  • Self-assessment questions
  • A reading
  • An audio-enhanced presentation
  • A case study and video vignette
  • Additional resources
  • Final assessment questions

Evaluation: At the end of the module, you will answer several multiple-choice examination questions related to the course objectives.  You will also be asked to complete two short evaluations – one is a survey built into the course content and the other is an evaluation of the overall training.

Dr. Margaret Gadon is a public health physician with experience in community health and health policy and holds an adjunct appointment at the Northwestern University School of Medicine. She is currently working as Regional Director for the Clinton Foundation in Ethiopia on health system reform. Prior to that, she directed a program in chronic disease and aging at the American Medical Association where she developed educational materials for physicians on self-management support and chronic disease management with a focus on vulnerable populations.
