Technical Support FAQ

1. What support is available to assist CMS-selected practices in qualifying to participate in the Medicare Medical Home Demonstration (MMHD)?
There are a number of tools available to support practices seeking to participate in the MMHD. Tools include online training, published guides, and consultation services. For more details, go to the Tools & Assistance section of this website.

2. Is there a charge for the technical assistance provided to practices?
Much of the technical support is free to CMS-selected practices that are seeking NCQA qualification to participate in the MMHD, thanks to a grant from the John A. Hartford Foundation. Certain tools, such as an implementation manual and individual practice consultation, do incur some charges to the physician practice.

3. How long will this MMHD assistance be available to practices? Assistance will be available through June 2011.

4. What can a practice do to determine if it is ready to become a medical home or participate in the demonstration project?
TransforMED offers a free, online assessment tool that helps practices to self-assess their readiness to become qualified as Medicare medical homes.

The American College of Physicians (ACP) has a practice support resource that provides an online evaluation of a practice's readiness to qualify as a medical home ("Practice Biopsy") linked to resouces selected based on the 'biopsy" results. [A link will be posted as soon as it is available.].
