Technical assistance for becoming a medical home is being provided by faculty and staff of the Roger C. Lipitz Center for Integrated Health Care at Johns Hopkins Bloomberg School of Public Health in association with the American Academy of Family Physicians, the American Board of Internal Medicine, the American College of Physicians, the American Geriatrics Society, the Center for Teaching and Learning with Technology at the Johns Hopkins Bloomberg School of Public Health, the Institute for Johns Hopkins Nursing, and TransforMED.

Chad Boult, MD, MPH, MBA, directs the Lipitz Center and is leading this effort. A geriatrician for more than 20 years, he is an expert in chronic care and has extensive experience in developing, testing, evaluating and diffusing new modules of health care.
